Two things that have helped for me, though not solved the problem: at least 10mg of melatonin, ignore that "start on 2mg" advice because it won't do shit. Second, Phenergan, aka promethazine hydrochloride.
Having added a chronic illness to my list of problems has pretty much decimated their effect on me, but when I was only unable to sleep because of my other meds, they helped.
